Navigating Dementia Support Resources in Halton Region: A Complete Guide for Burlington, Milton, and Oakville Families

Navigating Dementia Support Resources in Halton Region: A Complete Guide for Burlington, Milton, and Oakville Families

When families across Oakville, Burlington, and Milton face a dementia diagnosis, the journey ahead can feel overwhelming, but here’s the good news: you’re not alone.

Halton Region offers one of the most comprehensive dementia care ecosystems in Ontario, and knowing how to navigate these resources makes all the difference.

That’s where Always Best Care of Oakville comes in, serving as your trusted local guide through every step of this journey.

Getting Started: Diagnostic Services and Assessment

Halton Healthcare’s Geriatric Services

Halton Healthcare operates geriatric assessment clinics at three hospital locations: Oakville Trafalgar Memorial Hospital, Milton District Hospital, and Georgetown Hospital.

The Geriatric Assessment Clinic provides comprehensive evaluations through an interdisciplinary team consisting of geriatricians and nurse practitioners. These assessments are covered under OHIP, meaning families can access professional diagnostic services without out-of-pocket costs.

What sets these clinics apart? They create comprehensive care plans. The team evaluates multiple complex medical problems common in seniors, providing follow-up care and coordinating with community resources.

Medical referrals are required to access these services, so families should start by speaking with their family physician about memory concerns.

Specialized Diagnostic Services

For families seeking comprehensive neurological assessments, the Mississauga Institute for Neurological Disorders and Stroke (MINDS) provides specialized diagnostic services

The Alzheimer Society of Brant, Haldimand Norfolk, Hamilton Halton: Your Essential Partner

The Alzheimer Society of Brant, Haldimand Norfolk, Hamilton Halton serves as the region’s cornerstone resource for dementia education, support, and programs, and here’s the best part: services are completely free of charge.

Comprehensive Education Programs

The Society provides structured learning opportunities for every stage of the dementia journey:

  • First Steps introduces newly diagnosed families to Alzheimer’s disease and related dementias, covering daily management strategies over four weeks.
  • The Dementia Education Series focuses on planning for the future and supporting independence for as long as possible, also running for four weeks.
  • The Middle Stage Education Series addresses the unique challenges of mid-stage dementia, including responsive behaviours and home safety considerations, delivered over three weeks.
  • Care in the Later Stages prepares families for late and end-stage dementia realities through a three-week program.
  • The Reitman Centre CARERS Program represents the Society’s most comprehensive offering—an eight-week intensive program focusing on practical caregiving skills and emotional support strategies.

Community Wellness Programs

Beyond education, the Alzheimer Society offers engaging programs that promote both physical and cognitive health:

  • Minds in Motion combines physical activity with cognitive exercises in a dementia-safe environment, helping participants maintain function while enjoying social connection.
  • Creative Expressions leverages the arts to bypass affected brain areas and provide new avenues for emotional expression. The Society even hosts an annual Creative Expressions Art Exhibition showcasing the artistic talents of individuals living with dementia.
  • Sit to be Fit focuses on strength, endurance, flexibility, and balance through adapted exercise programs suitable for varying mobility levels.

These programs run year-round at various Halton locations, with both in-person and virtual options available.

Getting Connected with the Alzheimer Society

Families can access program calendars, register for events, and sign up for the Society’s newsletter directly through their website at alzda.ca.

The Society maintains a strong community presence with fundraising events, including the annual IG Wealth Management Walk for Alzheimer’s, which brings together families, volunteers, and supporters throughout the region.

Where Always Best Care of Oakville Fits In

Now, here’s where everything comes together. Always Best Care of Oakville doesn’t replace these institutional services; we complement and enhance them, serving as the bridge between fragmented resources and cohesive, personalized care.

The Local Expert Advantage

Owner Steve Kelly and the Always Best Care team possess in-depth knowledge of every resource mentioned in this guide. More importantly, we understand how to navigate between them, when to access which services, and how to create care plans that integrate multiple support systems.

We work collaboratively with organizations like Halton Healthcare, the Alzheimer Society, Acclaim Health, Ontario Health at Home, and specialized service providers throughout the region, but we provide something these institutions can’t: consistent, personalized, one-on-one support in the comfort of home.

Specialized Dementia Care at Home

Always Best Care of Oakville’s dementia care program goes beyond basic home care. Our approach includes:

  • Customized memory care activities designed for individual cognitive levels.
  • Assistance with personal care and hygiene using dignity-preserving techniques.
  • Medication management and reminders.
  • Nutritional support and specialized meal preparation.
  • Companionship and meaningful engagement.
  • Respite for family caregivers who need breaks.
  • Safety monitoring and fall prevention strategies.

Our caregivers receive comprehensive, ongoing training in dementia care techniques, including validation therapy, redirection strategies, and creating anxiety-reducing environments.

The “Meaningful Day” Philosophy

We begin every client relationship with a comprehensive assessment that goes beyond medical needs. Our “Meaningful Day” plan equips families with tools and resources to better understand and manage behavioural changes throughout the dementia journey.

Each client has a dedicated Care Manager who ensures needs are met and keeps families informed every step of the way.

Seamless System Navigation

Perhaps our greatest value? We handle the complexity. While families juggle multiple appointments, coordinate with providers, and struggle to understand eligibility requirements, Always Best Care serves as the central point of contact, simplifying everything.

Need a referral to the Alzheimer Society? We’ll help. Wondering if your loved one qualifies for Ontario Health atHome services? We know the process. Curious about Behavioural Supports Ontario? We’ve worked with their teams. Looking for the right adult day program? We’ve toured them all and can recommend the best fit.
